J s noted Los Angeles indices reflect on 11/21/73 MARY HENSLEY submitted an application for review of birth and death records for the year 1970. HENSLEY stated the reason for the appli~ cation was to put together her family tree, but on being ins formed of the application fee, $2.00, stated she did not have the money and left the office. A residence address of 513 Chapala, Santa Barbara, contained on HENSLEY's order form was subsequently determined to be a nonexistent residence. On 11/30/73 HENSLEY returned to the Buyeau of Vital Statistics office stating she wanted to obtajh a birth certificate of a deceased individual, CYNTHIA SUE/MANKINS. When ordering the certificate HENSLEY used the name MARY SCHWARTZ and gave her residence as 1923 De La Vina, Santa Barbara. Subsequent in- quiry at the De La Vina address determined HENSLEY aka SCHWARTZ unimnown at that address.
