He treated her for this ankle on four occasions in his office, the last such visit having been on February 25, 1974, QA advised that the last time he saw her was on February 25, 1974, and he had not visited her at her home. recalled that on one of these visits, MARION WADE :had been accompanied to his office b male belieVed to be her boyfriend, physically 1s oyfrien then checked with his office help and advised the writer that the only descriptive data the girls in the office could remember is that this individual was no taller than was MBRION WADE, who had her foot in a cast at the time. ay: made available to the writer, the patient information sheet regarding MARTON WADE; review of this sheet failed to disclose any information nét developed by prior investigation other than that MARION WADE: had listed her telephone number as $299127- and had°also lis€éd telephone number oe ebe as a "Message phone".
