SERVED WITH THE SUMMONS AND COMPLAINT ON ALL DEEENDANTS..
ADDITIONAL COPIES OF THIS NOTICE HAVE BEEN PROVIDED. FOR
THAT PURPOSE.
In accordance with ‘the provisions ‘of 28 Us Ss. C..
§636(c), you are hereby, notified that the full-time United
States Magistrates of this District Court, in addition to
their. other. duties, may, upon the consent of all parties.
to. their civil case, conduct: any. and all proceedings in’ a
civil case, including: a jury or non-jury trial, and. order
the entry of a final judgment: . Copies. of appropriate:
consent forms for this purpose (Form number CV-11) are
available from the Clerk of Court.
Your. opportunity to have your casé disposed: of -by a
Magistrate is subject t6. the calendar requirements of the.
court. Accordingly, the district judge to whom your case.
is assigned must approve the reference of the case to a
Magistrate for disposition.
You should be aware that your decision to consent, or
not to consent, to the referral of your case to a United
States Magistrate for disposition is entirely voluntary
and should be communicated solely to the clerk of the
district court by submitting form number CV-1l after it
has been. completed... ‘only if all parties to the case
consent to the rferencée to'a Magistrate will either the
Judge or Magistrate to whom the case has been assigned be
informed of your decision.
The parties may stipulate to the designation. of a
specific Magistrate to conduct all further proceedings. A
Space is provided on the consent form for use by parties if
they desire to stipulate to a specific Magistrate.
