INSTRUCTIONS (Detailed instructions are set forth in the NCIC Operating Manual) Accurately complete form in legible hand printing. A separate form must be completed for each entry. entering data, except when entering field office file number and miscellaneous information where such symbols are permissible. Use appropriate NciC codes, if known, otherwise fill in blank lines where they appear. STOLEN ARTICLE ENTRY An article having a serial number and/or an owner - applied number and meeting the criteria set forth in the NCIC Operating Manual may be entered in an NCiC single article record. Consecutively serialized artieles meeting the criteria set forth in the NCiC Operating Manual may be entered in an NCiC group article record. Where theft involves a group of articles which are identical except for nonconsecutive serial numbers, complete one form and attach a list of the nonconseeutive serial numbers and/or owner-applied numbers. 1. Check EA block indicating entry is a stolen article or EAA biock indicating entry is a group of consecutively serialized stolen articles. 2. Field Office Identifier (ORl) - Enter field office identifier. Type of Article (TyP) - Enter appropriate code or clearly print a description of the article on line provided, e.g. electric typewriter; color television; electric adding machine; single lens reflex camera; 16 mm movie projector; etc.. 4. Serial Number (SER)- Fill in spaces, starting with first space on the left, with the complete serial number including any letters which are part of the number. Caution: Enter serial number of the article, not the number of a component part of the article.. (a) When entering a single article, fill in first serial number block oniy. (b) When entering a group of consecutively serialized articles, enter the beginning serial number in the first serial number block and ending serial number in the second serial number block. 5. Brand Name (BRA) - Enter appropriate code or clearly print the brand name of the article on line provided, 6. Model (MoD) - Indicate the model of the article in the following order of priority: model number, model name, style or design, size, construction material.. Owner- Applied Number (OAN) - Fill in spaces, starting with first space on left, with the complete Owner-applied number including any letters which are part of the number. Caution: Owner- 8. Date of Theft (Dot) - Must be complete month, day and year, expressed in six numerals, e.gMay71967050767 9. Field Office File # (OcA) - Enter field office file number (include dash), e.g. 52-12345 10. Miscellaneous (Mis) - Clearly print any additional descriptive information in easily readable 11. NCIC # (N!C) - Leave blank. Will be recorded by terminal operator. STOLEN/MISSING OR *RECOVERED" GUN ENTRY Only serially numbered guns may be entered in NCIC. Where theft involves a group of guns which are identical except for non-consecutive serial numbers, complete one form and attach a list of the non-consecutive serial numbers, A recovered gun for which no lost or stolen report is on file in NCIC may be entered as a *recovered" gun. 1. Check EG block indicating entry is a stolen/missing gun or ERG block indicating entry is a "recovered* gun 2. Field Office Identifier (ORl) - Enter field office identifier. 3. Serial # (SeR) - Starting with first space on the left fill in spaces with the complete serial number. (Insure that any letters which are part of the number have been listed). Where more than one serial number appears on a firearmenter the frame number as the serial number. Make (MAK) : Enter appropriate code or clearly print the make of the gun on line provided. Refer to NCIC Operating Manual for assistance in entering U.S. military, U.S. Governmentand firearns of unknown make. 5. Caliber (CAL) - The appropriate gauge (or bore) should be entered as caliber of a shotgun: Decimal points should not be used,e.g. Caliber .32-- code as 32; Caliber 7.62 mm -- code as 762; 16 gauge - code as 16;etc. 6. rifle, bolt action; shotgun, double barrel; pistol, automatic;pistol, revolver, etc. 7. e.g.May11967050167 8. Field Office File # (0cA)- Enter field office file number (include dash), e.g. 15-2357 9. Miscellaneous (mis) - Clearly print any additional descriptive information, including barrel length, when known. Do not use periods for punctuation. Spaces, commas, dashes, etc. may be used.. Miscellaneous data is not allowed when entering a recovered" gun. 10. NCIC # (NIC)- Leave ill be recorded by terminal operator..
