copy of a statement, dated May 3, 2oo7, setting forth the threat

Elizabethtown, Kentucky. In summary the report indicated that
same date, captioned subject, who identified himself to

was being treated by Dr.

Based upon the above statement, on May 3, 2oo7,.
charged with Menacing, a class B Misdemeanor.
